Beacon, Saint Joseph and Franciscan Health Systems are partnering to host a FREE COVID-19
vaccine clinic for first, second, and booster Pfizer vaccines. Anyone 5 years of age and older
are eligible to receive first and second doses. Anyone 12 years of age and older are eligible

What to bring: Vaccines are FREE and you do not need insurance to receive one, but if you do
have insurance, please bring your insurance card. Please bring a form of ID and your vaccine card.
